ÖZET
Sakrospinöz ligament fiksasyonu (SSLF) vaginal prolapsus durumunda uygulanan bir ameliyat tekniğidir. SSLF, genellikle vaginal bir prosedürle uygulanmaktadır. Sunulan olgu ise abdominal olarak transperitoneal yolla SSLF gerçekleştirilen bir olgudur. Hasta 67 yaşında, uterusu grade 3 prolabe ve bilateral kalça osteoartriti mevcuttu. Histerektomi ardından vaginal kubbenin sağ posterolateralinden arka periton kesilerek retroperitoneal alana girildi. Sakrospinöz ligament/musculus coccygeus kompleksi palpe edildi. Ligamentten ve vaginal kubbe köşesinden geçildi, sütür tespit edildi. Her ne kadar vaginal prolapsus cerrahisinde abdominal yolu tercih eden cerrahlar bulunsa da halen çoğunlukla vaginal yol tercih edilmektedir. Abdominal transperitoneal yaklaşım ise uygun olgularda vaginal yolla SSLF'nin bir alternatifi gibi görülmektedir.
ABSTRACT
Sacrospinous ligament fixation (SSLF) is an operation technique that applied in cases of vaginal prolapse. SSLF is usually carrying out by vaginal route. In the presented case, SSLF was performed by abdominal transperitoneal course. The patient was 67 years old; uterus was grade 3 prolapsed and had bilateral hip osteoarthritis. After performing hysterectomy, posterior peritoneum cut through and entered into the retroperitoneal area. Sacrospinous ligament/musculus coccygeus complex palpated. The sutures passed through the ligament and vaginal cuff corner and fixed. Although some surgeons prefer abdominal way, vaginal course is still most preferred route in vaginal prolapse surgery. Abdominal transperitoneal style seems to be an alternative of vaginal SSLF in appropriate cases.
