Uterine Sarcomas: Analysis Of Histopathologic Features And Patterns Of Recurrence Of 53 Cases

ÖZET
İstanbul Üniversitesi Cerrahpaşa Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalı ile Radyasyon Onkolojisi Anabilim Dalı'nda 1978-1988 yılları arasında tedavi edilen 53 uterus sarkomu olgusu çalışma kapsamına alındı. Olguların histopatolojik dağılımı: 24 (%45.3)'ü leiomyosarkom, 14 (%26.4)'ü endometrial stromal sarkom, 7 (%13.2)'si mikst mezodermal tümör, 7 (%13.2)'si indiferansiye sarkom 1 (%1.9)'i rabdomyosarkom şeklinde idi. Hastaların 35'i evre I'de, 3'ü evre II'de, 9'u evre III'de ve 6'sı evre IV'deydi. Olguların 5'ine cerrahi, 43'üne cerrahi+radyoterapi uygulanırken 5 olguya sadece radyoterapi uygulandı. Leiomyosarkom olgularının %55'inde, endometrial stromal sarkom olgularının %50'sinde, miks mezodermal tümör olgularının %25'inde ve indiferansiye sarkom olgularının %0'ında en az iki yıl pelvik nüks ve uzak metastaz tespit edilmedi. Aynı süre içinde hastalığın evrelere göre tekrarlamama oranları sırasıyla, evre I'de %59.2, evre II'de %33.3, evre III'de %0 ve evre IV'de %0 idi. İlk iki yıl içinde hastaların %28.9'unda pelvik nüks meydana gelirken, %25'inde uzak metastaz tespit edildi.
ABSTRACT
Fiftythree patients with sarcoma of the uterus who has been treated at the department of Obstetrics and Gynecology and Radiation Oncology of Cerrahpaşa Medical Faculty between the years of 1978-1988 has been evaluated retrospectively. The histological types included 24 (45.3%) leiomyosarcomas, 14 (26.4%) endometrial stromal sarcomas, 7 (13.2%) mixed mesodermal tumor, 7 (13.2%) indifferentiated sarcomas and one (1.9%) rhabdomyosarcomas thirtyfive patients had stage I diseases whereas 3 had stage II, 9 had stage III and 6 had stage IV. Five patients have been treated with surgery, 43 with surgery plus radiotherapy and 5 with radiotherapy alone. Two years disease free survival rate was 55% for leiomyosarcoma, 50% for endometrial stromal sarcoma and 25% for mixed mezodermal tumor. There was no patient free of disease at least for two years in indifferentiated sarcom. The two years disease free survival rate was 59.2% for stage I and 33.3% for stage II and no patient with stage III and stage IV diseases survived at least two years free of diseases. Pelvic recurrences occured in 28.9% and distant metastases in 25% of the patients during first two years.
