ÖZET
Amaç: Atipik vulva lezyonlarında biyopsi yapmanın önemini göstermek. Bu lezyonlar, vulvar melanom da dahil olmak üzere, malign olabilirler. Olgu: Tek şikayeti vulvar bölgede 20 gündür farkettiği pigmentasyonu olan bir hasta. Lezyondan yapılan biyopsi sonucu vulvar nodüler malign melanom olarak saptandı. Hastaya tümörün lokal geniş eksizyonu ile birlikte bilateral inguinal lenf nodu disseksiyonu yapıldı. Operasyon sonrası hasta birinci yılında ve herhangi bir kötüleşme olmadan yaşamaktadır. Sonuç: Asemptomatik bile olsa vulvadaki tüm atipik lezyonlardan biyopsi alınması gereklidir. Vulvar malign melanomu olan hastalarda prognoz kötüdür, bu nedenle erken teşhis bu hastalarda çok önemlidir.
ABSTRACT
Objective: To show the importance of biopsy in atypical lesions of the vulva. These lesions could be malignant, including vulvar melanoma. Case: A patient with the only complaint of vulvar pigmentation for 20 days. Biopsy of the lesion revealed nodular malignant melanoma of the vulva. Local wide excision of the tumor with bilateral inguinal lymph node dissection was performed to the patient. Histopathologic examination after the operation showed 7 mm vertical invasion of the tumor without any satellite lymph node involvement. She is at the first year after the operation without any relapse. Conclusion: Biopsy should be performed in every atypical lesion of the vulva even it is asymptomatic. Patients with vulvar malignat melanoma has poor prognosis, therefore early diagnosis is imperative in these patients.
