ÖZET
Amaç: İlk trimesterde abortus imminens nedeniyle başvuran hastalarla herhangi bir şikayeti bulunmayan normal sağlıklı gebelerde human koryonik gonadotropin, progesteron, CA-125, tiroid hormonları ve protrombin zamanı, aktive parsiyel tromboplastin zamanı, total immunglobülin G, total immunglobülin M değerlerinin karşılaştırılması ve bunların prognozu belirlemedeki etkinliğini araştırmak. Çalışmanın Yapıldığı Yer: Selçuk Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum AD, Konya. Materyel ve Metod: 1.1.1999 ile 31.12.1999 tarihleri arasında Selçuk Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Kliniğine başvuran 46 abortus imminens teşhisi konulan gebe ve kontrol grubu olarak gebelik yaşı 12 haftadan küçük 70 sağlıklı gebe çalışmaya alındı. Gebelerin ilk başvurularında serum human koryonik gonadotropin, CA-125, progesteron, serbest triiyodotronin, serbest tiroksin, tirotropin, protrombin zamanı, aktive parsiyel tromboplastin zamanı, total immunglobülin G, total immunglobülin M seviyeleri tespit edildi. Gebeler doğuma kadar düzenli aralıklarla takip edildi. Gebelik sonuçları karşılaştırıldı. Bulgular: Kontrol grubu ile abortus imminens grubu karşılaştırıldığında yaş, gebelik sayısı, gebelik haftası, gebelik sonucu ve human koryonik gonadotropin, progesteron, CA-125, serbest triiyodotronin, serbest tiroksin, tirotropin,protrombin zamanı, aktive parsiyel tromboplastin zamanı, total immunglobülin G, total immunglobülin M değerleri arasında anlamlı farklılık bulunmadı (p>0.05). Doğum sonuçlarına göre sağlıklı doğum yapanlarla gebeliği spontan abortus, missed abortus veya in utero exitus ile sonuçlananlar karşılaştırıldığında human koryonik gonadotropin ve progesteron değerleri arasında ise istatistiksel açıdan anlamlı farklılık tespit edildi (p<0.05). Diğer parametrelerde anlamlı farklılık bulunmadı (p>0.05). Sonuç: Gebeliğin prognozunun belirlenmesinde en uygun parametrelerin human koryonik gonadotropin ve progesteron seviyeleri olduğu sonucuna vardık. Bu konuda daha fazla ayrıntılı araştırmanın gerekli olduğunu düşünüyoruz.
ABSTRACT
Objective: Examination of human chorionic gonadotropin, progesterone, CA-125, thyroid hormones, protrombin time, activated partial thromboplastin time, total immunglobulin G and total immunglobulin M values in first trimester pregnant women with threatened abortion and comparision of these levels with healthy pregnant women to see if any prognostic value of these tests. Institution: Selçuk University, Medical Faculty, Department of Obstetrics and Gynecology, Konya. Materials and Methods: 46 pregnant women hospitalized due to threatened abortion in our clinic between 1.1.1999 and 31.12.1999 were taken into study. As a control group 70 healthy pregnant women with <12 gestational age were taken. At admission, human chorionic gonadotropin, progesterone, CA-125, free triiodothyronine, free thyroxine, thyroid stimulating hormone, protrombin time, activated partial thromboplastin time, total immunoglobulin G and total immunoglobulin M levels were measured. Pregnant women were followed regularly up to birth and results were compared with each other. Results: When compared with control group, there was no statistically important difference between age, number of gestation, gestational age, results of pregnancy and human chorionic gonadotropin, CA-125, free triiodothyronine, free thyroxine, thyroid stimulating hormone, protrombin time, activated partial thromboplastin time, total immunoglobulin G and total immunoglobulin M levels in study group (p>0.05). According pregnancy outcome, pregnancy resulting spontaneous abortion, missed abortion or intrauterin death of the fetus compared to healthy ones. There was statistically significant difference in human chorionic gonadotropin andprogesterone levels between two groups (p<0.05). No significant difference was observed between two groups when the other parameters were compared (p>0.05). Conclusion: The favorable parameters for prognosis of pregnancy in patients with threatened abortion were found to be human chorionic gonadotropin and progesterone levels. According to our opinion more detailed research are necessary for the evaluation of pregnancy prognosis.
