Contraceptive Methods Used By Women Between 15-49 Years Old And The Cause Of Not Using Contraceptive Methods In The Semiurban And Urban Areas In Ankara

ÖZET
Amaç/Anafikir: Bu araştırmada, Ankara İlinde yarı kentsel ve kentsel bölgelerden seçilen "15-49 yaş Kadın İzlem Formları"nın 1999 yılı kayıtlarının incelenerek, kullanılan aile planlaması yöntemlerinin neler olduğu ve aile planlaması yöntemi kullanmayanların kullanmama nedenlerinin belirlenmesi amaçlanmıştır. Materyal-Metod: Şubat 2000 tarihinde yapılan bu araştırma tanımlayıcı bir araştırmadır. Araştırmada Ankara ili Gölbaşı İlçe Merkezinde bulunan Merkez I ve Merkez II Sağlık Ocaklarına ait formların tamamı (3094 adet), Yenimahalle İlçesi Yeşilevler Sağlık Ocağı ve Demetevler 1 Nolu AÇSAP bölgelerinden ise toplam 27875 formdan, sistematik örneklem yöntemi ile belirlenen 2000er adet form (toplam 4000 adet) incelenmiştir. Gölbaşındaki sağlık ocakları yarı kentsel bölge, Yenimahalledeki sağlık ocakları ise kentsel bölge olarak sınıflandırılmıştır. Aile planlaması yöntemleri, geleneksel ve modern yöntemler olarak ayırt edilerek, hangi yöntemin kullanıldığı ve aile planlaması yöntemi kullanmayan kadınların yöntem kullanmama nedenleri belirlenmiştir. Bulgular: Araştırmada incelenen yöntem kullanan kadınlardan yarı kentsel bölgede oturanların %75.7si, kentsel bölgede oturanların %67si modern yöntem kullanmaktadır. Yöntem kullanan kadınlar arasında yarı kentsel bölgede ve kentsel bölgede en çok tercih edilen modern yöntem RİA, ikinci sırada tercih edilen ise kondomdur. Yarı kentsel ve kentsel bölgede en çok yöntem kullanmama nedeni, 15-24 yaş grubunda çocuk istenmesi olarak belirlenmişken, 25-29 yaş grubunda yarı kentsel bölgede gebe olmak, kentsel bölgede yine çocuk istenmesi olarak belirlenmiştir. 30-34 yaş grubunda ise tersine, yarı kentsel bölgede çocuk istemek, kentsel bölgede ise gebe olmak en çok aile planlaması yöntemi kullanmama nedeni olarak görülmektedir. 35-39 yaş grubunda yarı kentsel bölgede diğer nedenler (menapoz, histerektomi, tıbbi nedenler v.b), kentsel bölgede çocuğu olmamak, 40-49 yaş grubunda ise her iki bölgede diğer nedenler en çok yöntem kullanmama nedeni olarak belirlenmiştir. Sonuç: İncelenen kadınlarda, aile planlaması yöntemlerinin en çok kullanıldığı yaş grupları, en çok kullanılan yöntemler ve yöntem kullanmama nedenleri, Türkiye geneli ile uyumludur ve yarı kentsel ve kentsel bölgede birbirine benzerdir. Modern yöntemlerin kullanımı ise Türkiye geneline göre daha yüksektir.
ABSTRACT
Objective: The aim of this study is to compare the methods of contraception used by women between 15-49 years old living in a semi-urban and urban areas of Ankara, with regard to 1999 records of the selected follow-up forms of women between age 15-49. The reasons for not using the contraceptive methods were also investigated. Method: The study, conducted in February 2000, was a descriptive research. Two primary health-care centers of Gölbaşı were selected and all the follow-up special document forms (3094 form) of women between 15-49 years old were investigated. A primary healthcare center and a Child Health Care-Family Planning Center of Yenimahalle were selected. 4000 follow-up forms which were selected by systematic sampling out of 27875 forms. Gölbaşı District was classified as a semi-urban area, where as Yenimahalle District was classified as a urban area. Contraceptive methods were classified as traditional and modern methods. Preferred contraceptive methods and reasons for not using contraceptive methods were determined.Results: In this study among the women using contraceptive methods, 75.7% of women living in semi-urban area were using modern contraceptive methods, where as 67% of the women living in urban area were using modern contraceptive methods. Among the women using the contraceptive methods IUD was the most preferred method while condom was the second preferred method in both semi-urban and urban areas. The reasons for not using contraceptive methods were child expectancy in semi-urban and urban areas among the women between 15-24 years old; being pregnant in semi-urban area and child expectancy in urban area among the women between 25-29 years old; child expectancy in semi-urban area and being pregnant in urban area among the women between 30-34 years old. Other reasons were menopause, hysterectomy, etc. in semi-urban area; bareness in urban area among the women between 35-39 years old. The other reasons (menopause, hysterectomy, etc.) among the women between 40-49 years old were determined as most common reasons for not using the contraceptive methods, in both areas. Conclusion: In this study, the age group using contraceptive methods, the most preferred contraceptive methods and reasons for not preferring contraceptive methods are highly correlated with the data of the Demographic Health Survey of Turkey (1998) and the results are similar in semi-urban and urban areas. The rate of using any modern contraceptive methods is higher than the data of The Demographic Health Survey of Turkey (1998).
