Correlation of Preoperative Diagnosis and Postoperative Histopathologic Findings in Patients Operated for Presumed Leiomyoma and the Increased Accuracy Rate in Preoperative Diagnose

ÖZET
Amaç: Bu çalışmanın amacı, bimanuel pelvik muayene ve ultrasonografik inceleme yöntemleri kullanılarak preoperatif miyom uteri ön tanısı ile opere edilen olgularda, postoperatif histopatolojik tanıdaki doğruluk oranlarının karşılaştırılmasıdır. Gereç ve Yöntemler: Pelvik muayene ve abdominal ve/veya transvajinal USG (TV USG) incelemeleri sonucunda uterin leiomiyom ön tanısı ile cerrahi tedavi uygulanan 602 olgunun tıbbi kayıtları retrospektif olarak incelendi. Ayırıcı tanı amacıyla ileri görüntüleme teknikleri (tomografi ve manyetik rezonans) kullanılan olgular çalışmaya dâhil edilmedi. Beş yüz yetmiş yedi olgunun ameliyat şekli ve postoperatif histopatolojik tanıları kaydedildi. Postoperatif histopatolojik inceleme ile doğrulanan leiomiyom tanısı alan olgu sayısı hesaplandı ve eş zamanlı diğer patolojiler (adenomiyozis, endometriyozis, diğer endometriyal patolojiler, diğer uterin korpus patolojileri, serviks patolojileri, over-tuba patolojileri) incelendi. Bulgular: Olguların yaş ortalaması 44.3 ± 7.5 yıl idi. Histopatolojik inceleme olgularında, %94.5'inde leiomiyom tanısı kesinleştirilmiştir. Leiomiyom saptanmayan 32 olgunun 9'unda servikal kronik inflamasyon dışında herhangi bir patoloji saptanmamıştır. Yirmi üç olgununn 6'sında, kendisi cerrahi endikasyon taşıyan, kitle görünümlü, leiomiyomu taklit eden patolojiler (leiomiyosarkom, malign potansiyeli bilinmeyen düz kas tümörü, granüloza hücreli tümör, endometriyoma) olduğu görüldü. Kalan 17 olguda ise neden oldukları belirti ve bulguya göre, cerrahi tedavi gerekliliği tartışmalı patolojiler (adenomiyozis, atipisiz endometriyal hiperplazi, ovarian stromal hiperplazi) mevcut idi. Leiomiyom saptanan olguların %41.8'inde leiomiyom dışında ek patolojik tanılar adenomiyozis (n= 60), endometriyozis (n= 15), diğer endometriyal patolojiler (n= 96), diğer korpus patolojileri (n= 6), over-tuba-periton patolojileri (n= 39), serviks patolojileri (n= 6) mevcuttu. Üç olguda uterin sarkom saptandı. Sonuç: Preoperatif tanıda doğru algoritmik değerlendirme ve uygulamalar (pelvik muayene ve ultrasonografi) ile seçilmiş leiomiyom ön tanısı ile opere edilen olgularda %95 gibi yüksek bir oranda doğru tanıya ulaşılabilmektedir.
ABSTRACT
Objective: The aim of this study was to compare the accuracy rates of the postoperative histopathological diagnosis with the preoperative diagnose in patients were operated for presumed leiomyoma uteri using the methods of bimanual pelvic examination and ultrasonographic imagine. Material and Methods: The medical records of 602 cases, who were underwent surgical treatment with the prediagnosis of uterine leiomyoma based on the pelvic examination and abdominal and/or transvaginal ultrasonography (TV-USG) evaluation, were retrospectively reviewed. The patients, in whom the advanced image techniques (computed tomography and magnetic resonance) were used for differential diagnosis, were not included in the study. The type of surgery and the postoperative histopathological diagnoses were recorded. The number of cases, in whom confirmed the diagnosis of leiomyoma by postoperative histopathological examination, were calculated and the other pathologies (adenomyosis, endometriosis and other endometrial abnormalities, pathologies other uterine corpus, cervix abnormalities, ovarian-tubal pathology) were analyzed. Results: The mean age of the patients were 44.3 ± 7.5 years. As a result of histopathological examination, leiomyoma diagnosis was confirmed in 94.5% of the cases. There was no pathology other than cervical chronic inflammation in 9 cases out of 32 without leiomyoma. In 6 patients out of 23, pathologies with surgical indications, presenting as a mass, mimicking leiomyoma (leiomyosarcoma, malignant smooth muscle tumor with unknown potential, tumor with granulating cells, endometrioma) were observed. In the remaining 17 cases, pathologies with disputable necessity for surgery (adenomyosis, non-atypical endometrial hyperplasia, ovarian stromal hyperplasia) were present. In 41.8% of leiomyoma cases, there were additional pathologies such as adenomyosis (n= 60), endometriosis (n= 15), other endometrial pathologies (n= 96), other corpus pathologies (n= 6), ovarian-tubal-peritoneal pathologies (n= 39), cervical pathologies (n= 6). Uterine sarcoma was detected in 3 cases. Conclusion: In the patients, who were selected in the preoperative diagnostic process with the algorithmic assessments and applications (pelvic examination and ultrasonography) and operated for presumed leiomyoma uteri, the correct diagnosis rate can be reached as high as 95%.
