Subjective Results Of The Polypropylene Pubovaginal Sling For The Treatment Of Stress Urinary Incontinence

ÖZET
Amaç: İntrensek sfinter yetmezliği ile birlikte olan stres üriner inkontinans (SÜİ) slingler için alışılmış endikasyondur. Bu çalışmamızda kliniğimizde polipropilen mesh ile pubovaginal sling (PVS) uygulanan stres üriner inkontinanslı hastaların ameliyat öncesi değerlendirmesi ve ameliyat sonrası dönemde izlemleri sunulmuştur. Gereç ve Yöntemler: 1995-2002 tarihleri arasında SÜİ nedeniyle 29 hastaya PVS operasyonu yapıldı. Hastalar ameliyat öncesi dönemde öykü, fizik muayene, sistografi, sistometri ve SEAPİ sübjektif sorgulama formunun 1. sorusuna verilen yanıt ortalaması ile, ameliyat sonrası dönemde ise SEAPİ sorgulama formunun 1. sorusuna verilen yanıt ortalaması ve 5. sorusuna verilen yanıt ile değerlendirildi. Tüm hastalarda sling materyali olarak propilen mesh kullanıldı. Bulgular: Hastaların yaş ortalaması 54.1 ± 4.5 idi. 13 hastaya SÜİ nedeniyle farklı cerrahi teknikler daha önce uygulanmıştı ve 10 hastanın geçirilmiş jinekolojik operasyon öyküsü vardı. Hastaların sistometrik değerlendirmesinde ALPP (Abdominal Leak Point Pressure) 11 hastada 60 cmH2O'yun üzerinde, 18 hastada 60 cmH2O'yun altında bulundu. 24 hastaya sadece PVS uygulanırken, 5 hastaya sistosel onarımı ve PVS uygulandı. Ortalama hastanede kalış süresi 4.1 ± 1.9 gün idi. Ortalama takip süresi 16.3 ± 3.5 ay olarak gerçekleşti. Ameliyat sonrası dönemde SEAPİ subjektif sorgulama formları ile yapılan değerlendirmede 24 (%82.8) hastada tam inkontinans sağlanırken, 3 (%10.3) hastada de novo urge inkontinans, 2 (%6.9) hastada persistan SÜİ saptandı. Üretral ve vaginal erozyon izlenmedi. 3 (%10.3) hastada üriner retansiyon gelişti ve bu hastalardan birine transvaginal sling insizyonu uygulandı. Sonuç: PVS stres üriner inkontinans için başarı oranı yüksek ve cerrahi morbiditesi düşük alternatif bir tedavi seçeneğidir. Tarihsel olarak PVS uygulaması intrensek sfinkter yetmezliği veya önceki başarısız girişimleri olan vakalarda sınırlı kalmıştır. Çalışmaya alınan hastalarımızdan daha önce farklı prosedürler uygulanarak başarısız olunmuş 13 hasta ve diğer inkontinans tipi bulunan 11 hastada kontinansın sağlanması, PVS'in SÜİ'ın tüm tiplerinin tedavisinde de uygulanabilecek bir yöntem olduğunu göstermektedir.
ABSTRACT
Objective: Stress urinary incontinence (SUI) associated with an intrinsic sphincter deficiency is usual indication for slings. In this study, we analyze preoperative evaluation and postoperative follow-up of patient having pubovaginal sling (PVS) with polypropylene mesh. Material and Methods: Between 1995 and 2002, PVS was performed to 29 patients diagnosed with SUI. All patients were evaluated in preoperative period with history, physical examination, cystography, cystometry and average value of first question of SEAPI (Stress-related leakage, Emptying ability, Anatomy, Protection, Inhibition) subjective questionnaire and post operative period with average value of first question and respond to fifth question of SEAPI subjective questionnaire. In all patients, polypropylene mesh (marlex sling) was used as a sling material. Results: Mean age was 54.1 ± 4.5. 13 patients had previous surgical operations for SUI and 10 patients had history of gynecologic operations. ALPP (Abdominal Leak Point Pressure) was below and above 60 cmH2O in 18 and 11 patients, respectively. Only PVS was performed to 24 patients, in 5 patients PVS and repair of cyctocele were performed. Mean hospitalization was 4.1 ± 1.9 days. At a mean follow-up of 16.3 ± 3.5 months, evaluation with SEAPI forms in postoperative period revealed complete continence in 24 (82.8%), de novo urge incontinence in 3 (10.3%), persistent SUİ in 2 (6.9%). No uretral or vaginal erosions occurred. Three patients developed urinary retention for which one patient required transvaginal sling incision. Conclusion: Pubovaginal sling is a alternative treatment with low surgical morbidity and satisfactory results for SUI. Historically, PVS has been reserved for cases of intrinsic sphincter deficiency or prior surgical failure. Favorable outcome of 13 patients who previously had surgical operations for SUI and 11 patients having uretral hyper mobility reveals that PVS may be performed in all kinds of stress incontinence.
